Before you can get a card or meet the doctor, you must have one of these qualifying illnesses:

Spinal cord disease (including but not limited to arachnoiditis)

Spinal cord injury is damage to the nervous tissue of the spinal cord with objective neurological indication of intractable spasticity

3.
Arrive at your appointment with your medical records.
4.
After Dr. Jerabek verifies that you have the qualifying condition, we will fill out the necessary paperwork.
5.
Mail the paperwork.
6.
The State of Illinois will review your paperwork and your Medical Cannabis License will be mailed to the address listed in your application.
